PRE-OPENING INSPECTION. FACILITY HAS BEE USED FOR TEMPORARY EVENTS PRIOR TO GETTING SET UP TO BE FULLY LICENSED. FACILITY HAS ACQUIRED THE COMMISSARY FRO THE MOBILE UNIT. THE COMMISSARY IS ESSENTIAL SMOOTHIES AT THE MALL. THE COMMISSARY HAS BEEN REVIEWED AND APPROVED. LETTER OF AGREEMENT FOR SPACE USE WAS RECEIVED FROM THE COMMISSARY OWNER.
AFTER REVIEW OF THE FOOD PROCESS, FACILITY WILL BE BUMPED UP TO A CATEGORY 1( HIGH RISK). OWNER AGREED TO BE CHARGED THE EXTRA 50 FEE DIFFERENCE. WITH THE REVIEW OF THE FOOD HANDLING PRACTICES AND INSPECTION OF THE UNIT. THIS FACILITY IS GRANTED PERMISSION TO OPERATED BY THE KANKAKEE COUNTY HEALTH DEPARTMENT. RECEIPT OF ADDITIONAL CHARGE WILL BE EMAILED TO OWNER. |
